Self-Schedule an Appointment

Yale Medicine is pleased to provide the convenience of self-scheduling to new and returning patients.

New patients are those who are new to a specific doctor, not necessarily new to Yale Medicine.

• Click on Specialists in the main navigation to search by name, specialty, or condition.
• Once you have found a doctor you would like to see, click on that doctor’s name to go to their profile page, and click “Book Appointment.” Then fill out the form and follow the instructions.

Please note: Self-scheduling for new patient appointments is available for some, but not all, Yale Medicine doctors. And, in some cases, you may be told that a referral from your primary care physician is required, and you will be given a phone number to call for more information

• Click on the “Visits” tab at the top of the page, and choose “Schedule an Appointment.”
• You will then be given the choice of scheduling the appointment yourself, or sending a request for an appointment to your provider.
• If you are using the MyChart app on a mobile device, simply click the “Appointments” icon, and then “Schedule an Appointment.”
