Becoming an International Medicine Patient
Please follow these simple steps to submit a request as a prospective patient. Please be aware that you or your physician will need to submit your medical records, translated in English, as part of this process.

Step 1: Contact Us
Please complete our brief Information Request Form, to provide us with information about yourself and the medical services you are seeking. For assistance, please call us at +1-203-495-9253 Monday to Friday, between 8 am and 5 pm Eastern Standard Time. Interpreters are available for more than 50 languages.
Step 2: Submit Medical Records
An International Medicine Program Coordinator will contact you within 1-2 business days after the Patient Request Form is completed. The Coordinator will give you instructions to submit your medical records to a secure website. Your medical records must be translated into English.
Step 3: Decision and Estimate
Within 7 business days of receiving all medical records, our physicians will review your information. If we determine that the International Medicine Program is appropriate for your care, we will provide an estimate for hospital and physician expenses. You’ll also receive a financial guide that outlines all payment conditions and financial requirements.
Step 4: Payment
Payment for all charges is required prior to scheduling any medical appointments or assistance with travel arrangements. Your International Medicine Program Coordinator will work with you to process your payment. Once full payment is received, you will officially become a patient of the International Medicine Program, a collaboration between Yale Medicine and Yale New Haven Hospital.
