Transition Program for Young Adults with Chronic Rheumatic Conditions
The Transition Program for Young Adults is a joint program with Yale Pediatric Rheumatology that is designed to facilitate the transition from pediatric to adult care for young adults with chronic rheumatic conditions. We work with patients to understand their goals and help them become knowledgeable and independent in their health care decision-making.
Our Approach
Our Lupus Wellness Program provides patients with knowledge about their disease, and with the methods and means to manage multiple aspects of their health. Meeting in a one-on-one session with the nurse practitioner, patients will learn about how their disease affects the body and how to recognize and manage lupus symptoms. We also teach patients how to incorporate preventive health practices into their day-to-day life, and improve self-advocacy and adherence. The Lupus Wellness Program empowers patients with tools to help maximize communication with their health care providers and use technology to maintain their own personal health records. We also host support group meetings throughout the year where patients can meet other people with lupus and learn about nutrition, exercise, and stress reduction techniques together. This program is an especially important resource for those newly diagnosed with lupus or those who are transitioning into adulthood. The program is also offered in Spanish.
