B – Ordering testing for patients seen outside Yale New Haven Health Systems using our Sample Submission Form:
Download and submit a completed Sample Submission Form {link to “Yale DNA Diagnostics Laboratory Sample Submission Form”[pdf in email attachement]} along with your sample.
Shipping your patient’s sample from a site NOT affiliated with the YNHH system:
- Pack your sample securely in a strong polystyrene foam box with appropriate padding and the printed Sample Submission Form in a plastic bag.
- Packaging must indicate the nature of shipped materials, such as “biohazard” or “diagnostic specimens” labels.
- Samples MUST be shipped at room temperature. DO NOT FREEZE.
- Samples MUST be shipped by overnight mail to arrive on any weekday. Do not ship for arrival on Saturday or Sunday.

C - Special submission:
After 5 p.m., samples must be hand-delivered to the YNHH Core Laboratory.
Saturday or holiday delivery requires PRIOR special arrangements with the DNA Diagnostics Laboratory.
Please contact the DNA Diagnostic Laboratory (203-785-5745) 24 hours after shipping to confirm that we have received the sample or if you need any special arrangements.
