Sight-saving Engagement and Evaluation in New Haven (SEEN) Program

DIf you are 18 years of age or older, have diabetes, and have not had an eye exam in the last year, you may be eligible to participate in SEEN Pilot study. Study participants will receive a full eye exam, be enrolled into a patient navigator program and will also be supplied with diabetic educational materials.

Compensation will be provided.

    Trial Purpose and Description

    The primary objective of this study is to design and pilot a patient navigator program to increase screening for diabetic retinopathy (diabetic blindness) in a high-risk population.

    Eligibility Criteria

    Eligibility:

    • Adults aged 18 and over
    • Diagnosis of diabetes and record of diagnosis in YNHHS Epic
    • English-speaking
    • No documented eye examination within 1 year of study enrollment
    • High risk for diabetic retinopathy based on risk calculator evaluation
